Howdy Y'all!
Friday I received an email from Mirixa.
I haven't had time to open it yet.  It's like a present from Grandma that arrives a couple of days before my birthday.  I know there is something good inside.  If not, my Mommy will return it to Lord and Taylor.  Yup, that's where Grandma shopped!  
I will take this up again after I open the email from Mirixa.

The Appointment Part Deux

David blew off his appointment as I told you.  I was out of town again and could not reschedule with him until I returned.  By the time I came home and back to work Mirixa had removed his case from my queue.  Mirixa was very understanding.  After one quick call the case was back in my file and I called David to make an appointment for a telephone MTM.  I copied my papers from our MTM certification class and called David at the appointed hour.  He was ready willing and able to do our CMR and complete MTM.  He told me what he was taking and he knew the reasoning for each medication.  This is a patient that came to me initially from pain management.  I am not sure how he found our pharmacy except that my philosophy has always been to get the out of the ordinary items when I could.  He fills an RX for 120 Demerol injectable amps every month.  He has neuropathic pain.  Now he fills that, his diabetic drugs and cardiovascular meds with us as well.  After doing the CMR according to the procedure from our certification class I attempted to enter the info on the Mirixa site.

Now the fun begins.  The Mirixa CMR is a little more specific as to what data has to be entered.  This necessitated another call to David.  After all data was entered I printed out the Personal Medication Record and his Medication Action Plan.  I gave him a few suggestions like taking Simvastatin at bedtime instead of the morning and following up with his endocrinologist about a followup A1C.  As a followup I suggested that he and I get together so he could show me his self monitoring blood glucose testing technique.  I also had an issue with one medication that I neglected to mention in my submission to Mirixa. I made and addendum to my submitted data.  So far I have not had any subsequent emails about MTM cases from either Mirixa or Outcomes.  I was invited to speak to the senior group at our community center in West Hempstead.  I discussed MTM on a level easily understood by patients in addition to my prepared talk on Drug Interactions in the Elderly.  I also handed out coupons to the 50 or so attendees offering a free MTM session ($100 Value).  I listed my cell phone to make the appointment.  So far, NO TAKERS!!!

The Appointment

Good evening sports fans!
Yes I made the appointment with my patient that was assigned to me by Mirixa. I entered it on the Mirixa website with my son's help. He did a rotation at a MTM specialty pharmacy. NO, my patient did not make the appointment. He didn't even call to reschedule. Yes, you are right. I should have called him the night before to remind him. I wonder why? 
Right now I am at the PDS conference in Orlando. I will report any and all MTM stuff that I learn here to you. I am all about SHARING.

I can't hear you! Please raise your voice.

Good day to all my faithful MTM blog followers.  Do you remember I told you about the correspondence I received from Mirixa?  Two full blown MTM cases were offered to my pharmacy.  Unfortunately, one of the patients passed away.  I guess I should fudge one for her and see what happens.  Of course I wouldn’t do that!  I just wanted to see if you were listening!!  The other case is a man that I know well and really have no apprehension about meeting with him.  My pharmacist could not resist reminding me that "there is no way she can perform this task while she is working the bench filling prescriptions”.  Wah, Wah - stop your whining, I’LL DO IT.  I guess I will call him tomorrow and make the appointment to do a CMR.  What will I need?

The Federal government is making some changes to Medicare Part D.  These changes are positive for community pharmacists.  They address PBM issues, improved benefits and MTM expansion.  The PBMs won’t allow these changes without a fight so it is imperative that you voice your opinion.  Please follow the instructions below.

The Center for Medicare and Medicaid Services (CMS) is accepting public comment through Friday, March 7, 2014, in regards to the 2015 Medicare Parts C and Part D Proposed Rule. NCPA strongly supports a number of provisions in the proposed rule, including changes to Preferred Networks, increased MAC transparency, improved beneficiary cost sharing levels and expansion of MTM, and will be submitting comments on behalf of the association to reflect this position. 

We have been advocating for these changes for several years, and we are positive the PBMs won’t let these changes take effect without a fight. In order to counter the efforts of the PBMs it is imperative that you submit your own public comments on this important Part D Proposed Rule to CMS.

A sample letter is included at the end of this email which we ask you to personalize with your own experiences to support the points made. It is extremely important that you customize your submission with your comments and do not simply submit the information provided as-is. 

INSTRUCTIONS: These comments should be submitted electronically via You will need to search for RIN: 0938-AR37and then follow the "Comment Now” submission directions for "Medicare Program: Contract Year 2015 Policy and Technical Changes to the Medicare Advantage and the Medicare Prescription Drug Benefit Programs”. The best way to submit is attach a document (Word or PDF) so as not to be limited in wordage by the text box. The category you will be submitting under will be Health Care Professional/Association - Pharmacist. Comments will be accepted until 11:59 p.m. ET on March 7, 2014. Note: Do not include any patient-specific, HIPAA-protected information in your comments.

To Whom It May Concern:

I am writing in response to the release by CMS of the proposed changes to the Medicare Parts C and D programs for 2015. I applaud CMS for addressing certain issues in the Proposed Rule that have greatly concerned independent community pharmacies and our patients for many years. As an independent community pharmacist, I wish to voice my support for the following proposals that focus on eliminating abusive practices by Medicare Part D pharmacy benefit managers (PBMs) that have disadvantaged patients and independent community pharmacies. I respectfully ask that CMS implement the following proposals in the Final Rule:

Preferred Cost Sharing and Any Willing Pharmacy Standard Terms & Conditions:  I applaud CMS for formally recognizing that although the agency expected costs via preferred pharmacy networks to be consistently lower, CMS’ own findings proved otherwise, especially in mail-order claims. I also applaud CMS for formally recognizing that PBM’s strive to maximize profits by moving as much volume as possible to their own mail order facilities, with the ultimate outcome being little to zero savings in preferred pharmacy pricing, especially for mail order generic drugs, where costs to the government are more than via non-preferred community pharmacies like mine.   

I fully support CMS’ proposal to require Part D plan sponsors to offer terms & conditions for every level of cost sharing, including preferred cost sharing, to any willing pharmacy that will accept the terms.  This will afford my pharmacy the ability of at least seeing the contract terms and conditions, so I can make an informed decision to participate or not.  Most importantly, this proposal should benefit seniors by giving them more competition and choice among pharmacies in their drug plan. 

In addition, I support CMS limiting cost sharing levels offered to beneficiaries as this should help ease beneficiary confusion as well as allow patients to make choices based on customer service and ease of access rather than cost only. 

Prescription Drug Pricing Standards and Maximum Allowable Cost: I commend CMS for recognizing that Maximum Allowable Cost pricing is a "prescription drug pricing standard.”  When entering into take-it-or-leave-it contracts, I currently have no insight whatsoever into the process PBMs employ to set MAC prices. In addition, because of a lack in timely updates to MAC prices, I am frequently underpaid when drug prices spike, as is the case recently with many generic drugs.  More frequent updating should assist in alleviating this problem if the updates are based on verifiable current market conditions. 

It is encouraging that CMS expects pharmacies to have current data on the amount of reimbursement they can expect, which in turn impacts costs that plan sponsors submit to CMS as well as prices displayed on Plan Finder. I also support CMS including the phrase "includes, but is not limited to” when describing what constitutes a prescription drug pricing standard, as PBMs will inevitably shift to different reimbursement mechanisms over time.

Expansion of Medication Therapy Management Program (MTM) Under Part D:  Community pharmacists have been providing MTM related services for many years, to different patient populations, including Part D beneficiaries. These services are critical to patient understanding and adherence to their medication regimens, and are ideally provided face-to-face by a community pharmacist. 

I fully support CMS in the agency’s efforts to expand access to these critical services, as the variability in different Part D plan MTM offerings and limited compensation has made it difficult for me to provide these services in a consistent, scalable and timely manner. In addition, I am aware that many plans offer MTM via the phone only, by their own internal staff. I agree with CMS that MTM must become a cornerstone of the Prescription Drug Benefit, and for this to occur there must be more coordination with the community pharmacies that care for Part D patients and the local prescribers they work closely with.




The proposed rule takes substantial and very welcoming steps toward addressing Part D PBM abuses that have been plaguing beneficiaries and pharmacies. I commend CMS for recognizing the need for significant changes to the Part D program and I urge that the Final Rule retain these changes to protect the interests of beneficiaries, further promote pharmacy competition, and most importantly provide true patient choice.

Additional considerations for LTC pharmacies: 

·         As an independent pharmacy providing long-term care pharmacy services, I fully support CMS’ proposal to prohibit payment arrangements, such as prorated dispensing fees, that penalize efficient LTC dispensing techniques.  I greatly appreciate CMS recognizing, in the preamble to the rule, that "there is no justifiable rationale for proration, since the cost of dispensing is not directly related to the quantity dispensed.”

·         Even though CMS has required that MTM services be provided to LTC residents, in my experience the Part D plans are not offering LTC pharmacies the opportunity to provide the required interventions.

Welcome Message

You did it! You got certified in MTM. Don't you feel a sense of accomplishment? I know I did. That feeling lasted a good couple of days. It has since faded and has been replaced with that tinge of guilt for not having put that training and knowledge to use. My patients have not benefited health wise and my pharmacies haven't benefited financially. I hear there is a lot of potential for both. Can you add your thoughts to this blog? What are you doing in your practice setting with respect to MTM services? Lets share some ideas with one another. How did you start? When do you do it. Where do you get your cases? What marketing do you do? What hasn't worked for you? I think the writing is on the wall - we will be paid for cognitive services. 

Please share your ideas generously in this blog. Oddly enough I received an extensive email from Mirixa today which just upon opening has given me high anxiety. Pass me a Xanax please!
