AZ Medicine & Me™ for People in Medicare Part D Program is designed to provide significant savings to qualifying people who are enrolled in Medicare Part D prescription drug coverage plans and are having difficulty affording their AstraZeneca medicines. Enrollees will pay no more than $25 for a typical 30-day supply of AstraZeneca medicines. AZ Medicine & Me™ for People in Medicare Part D is special because it's easy to sign up, has no enrollment fee, and is available at most local pharmacies.

Individuals enrolled in Medicare Part D may be eligible for the program if they meet the following criteria:
Are an individual with an annual income below $30,000 or a couple earning less than $40,000 per year.
Are taking a listed AstraZeneca product.
Have spent at least 3% of their annual household income on prescription drugs this year.
AZ Medicine & Me™ for People in Medicare Part D offers a simple application process to help ensure that individuals promptly receive assistance. Starting November 15th, people can learn more about whether or not they or a family member may qualify for the program, by calling the AZ Medicine & Me™ hotline at 1-800-957-6285, Monday through Friday, 8:00 am to 8:00 pm EST.

Upon completion of the documentation requirements, people will have a choice of getting a 30-, 60-, or 90-day supply of medicines filled at their local pharmacy, which will include additional savings on the 60- and 90-day fills.
Program cost structure:
Individuals with annual incomes at or below 150% percent FPL:
$15 for a 30-day supply per medicine
$22.50 for a 60-day supply per medicine
$30 for a 90-day supply per medicine
Individuals with annual incomes between 150% percent FPL and 300%FPL:
$25 for a 30-day supply per medicine
$37.50 for a 60-day supply per medicine
$50 for a 90-day supply per medicine
