REMINDER: RPh's Legal Responsibility to the Bureau of Narcotic Enforcement - Pharmacists should be reminded that they are legally responsible to report all suspicious controlled substance activity to the Bureau of Narcotic Enforcement (formerly Bureau of Controlled Substances). Each instance or alleged incident of controlled substance theft, loss, prescription forgery, or possible diversion of controlled substances should be reported to the BNE Office in your region. If you suspect possible prescription forgeries, controlled substance thefts, loss or diversion of controlled substances you must report it:

January 28, 2010: Forged Prescription Alert: Please note that forged prescriptions are being used for controlled substances using the following Doctor's information. Please verify all prescriptions, especially those for narcotics: Thomas Diflo, M.D., FACS, Associate Professor of Surgery, Director, Surgical Skills Lab, Chairman, NYU Institutional Review Board C, NYU School of Medicine, 403 E34TH ST, Third Floor, New York, NY 10016, phone: 212-263-8134, fax: 212-263-815. A recent attempt to fill a prescription was made at a pharmacy in the Bronx.

January 28, 2010: Forged Prescription Alert: Devon G. John, MD is a practicing surgeon at New York University-Langone Medical Center, NY, NY, business address: 403 East 34th Street, Third Floor, NY, NY 10016. Yesterday someone tried to use one of his prescriptions to attain Oxycontin from a CVS pharmacy in Suffern, New York. The name listed on the prescription is Alfonso Elliot. (not sure of the correct spelling)
Please note that this Doctor does not prescribe Oxycontin and does not have a pt by that name. If you need to verify a prescription please contact the Dr's office directly at: Tele: 212-263-8134, Fax: 212-263-8157

November 16, 2009: Please note: a prescription pad has been stolen from Kaleida Health- Millard Fillmore Gates Circle Hospital, Buffalo NY. These are from the outpatient pediatric psychiatric unit. Rx Information is as follows:

The doctor advises that he only rarely writes for any controlled pain meds...

October 9, 2009: McNeil Consumer Healthcare, Division of McNEIL-PPC, Inc., is working with the U.S. Food and Drug Administration, Office of Criminal Investigations (FDA), and other law enforcement officials to recover cases of select lots of TYLENOL® Arthritis and TYLENOL® PM products that were stolen from a cargo terminal at the Jacksonville Port Authority in Jacksonville, Florida on September 25, 2009. The products stolen were cases of:

- TYLENOL® Arthritis Pain Caplet 150 count bottles (57,456 bottles)
- TYLENOL® PM Caplet 2 count packets (180,000 2 caplet pouches)

McNeil Consumer Healthcare, along with FDA and local authorities, has initiated these efforts in order to locate the stolen product and identify the individuals who were involved in the theft.

Consumers who received or purchased either of these products should check to see if they are from the stolen lots identified below.

UPC# 30300450482304 Code # 4823000 Lot # 09XMC110 Description: TYLENOL PM CAPLETS 2 COUNT PACKET

The stolen TYLENOL® Arthritis Pain Caplet product lot number can be found on the bottom of the outer box, and also on the bottle label. The stolen TYLENOL® PM product lot number can be found on the individual packet.

McNeil Consumer Healthcare and the FDA are asking for the public's help in reporting any information regarding the stolen TYLENOL® products to FDA's Office of Criminal Investigations (OCI) by calling 800-551-3989 or by visiting the OCI Web site (http://www.fda.gov/OCI).
Although McNeil is not aware of any reports of adverse events related to these stolen lots, consumers and retailers should return these products to McNeil Consumer Healthcare. For consumer inquiries about the product and how to return any product from the stolen lots, call McNeil Consumer Healthcare at (888) 222-6036.

September 21, 2009 - FDA is alerting you to a pharmaceutical theft of Albuterol Sulfate Inhalation Solution, 0.083%, unit-dose vials and Ipratropium Bromide Inhalation Solution, 0.02%, manufactured by DEY L.P., a subsidiary of Mylan Inc. The drugs were on a tractor-trailer that was stolen on September 8, 2009, in the state of Florida.

In the weeks prior to the truck theft, some portion of each lot had been sold by Dey to its customers and successfully entered the legitimate pharmaceutical supply chain. Because the stolen product may have been stored or handled improperly, Dey is requesting that any product with the affected lot numbers be isolated and not be dispensed, sold or used. Dey has alerted its distributors and customers.

FDA is committed to protecting the public from receiving stolen drug products that may not have been stored and handled properly and may be dangerous for patients to use.

In support of this mission, FDA is requesting assistance from pharmaceutical drug distributors and pharmacies that may receive offers for the stolen drug products described above.

August 31, 2009 - FDA is alerting you to a pharmaceutical theft of 14,400 units of Advair Diskus 250/50 oral inhaler and 11,200 units of Advair Diskus 500/50 oral inhaler that were stolen on August 2, 2009 from the GlaxoSmithKline distribution facility near Richmond, Virginia. FDA is committed to protecting the public from receiving stolen drug products that may not have been stored and handled properly and may be dangerous for patients to use.

In support of this mission, FDA is requesting assistance from pharmaceutical drug distributors and pharmacies that may receive offers for the stolen drug products described below. It is important to note that these lots were awaiting distribution to retail pharmacies at the time of the theft, therefore, these products should not be in the legitimate pharmaceutical supply chain.

If you receive any offers for stolen Advair Diskus products or think that you may have been sold any stolen Advair Diskus products referenced in this notice, FDA is asking your help in reporting this information to FDA's Office of Criminal Investigations (OCI) by calling 800-551-3989 or by visiting the OCI web site
(http://www.fda.gov/OCI).

Patients who have inhalers with these lot numbers should return them to the pharmacy where they were dispensed.

August 31, 2009 - Levemir Insulin (Novo Nordisk) REMINDER: FDA is reminding the public that stolen vials of the long-acting insulin Levemir made by Novo Nordisk Inc. still may be on the market. Evidence gathered to date suggests that the stolen insulin was not stored and handled properly and may be dangerous for people to use. FDA has received multiple reports of patients who suffered an adverse event due to poor control of glucose levels after using a vial from one of the stolen lots.

FDA is committed to protecting the public from receiving stolen drug products that may not have been stored and handled properly and may be dangerous for patients to use.

In support of this mission, FDA is requesting assistance from pharmaceutical drug distributors and pharmacies that may receive offers for the stolen drug products described below. It is important to note that every carton of Dey brand generic Albuterol Sulfate manufactured with the below lot numbers was subject to this theft and was never previously distributed, so none of this product should be in the legitimate pharmaceutical supply chain.

- Sanofi-Aventis suffered a tractor trailer theft of several drug products. FDA is committed to protecting the public from receiving stolen drug products that may not have been stored and handled properly and may be dangerous for patients to use.
