At the completion of this activity, the participant will be able to:

·        Discuss the frequency of occurrence of medication errors and the estimated costs to the health care system in the United States.

·        Define medication errors, and common types and causes that occur in pharmacies. 

·        Describe inattentional blindness and
ways to avoid errors in dispensing  

·        List the key elements of medication use and benefit associated with each.    

·        Recognize common errors in electronic prescriptions and identify the appropriate interventions needed.

·        Discuss the importance of liability insurance and the risk associated with errors.

·        Identify reference sources available to pharmacists for more information.
